1. Residents of Punjab. 1. Applicants should be from the following categories - SECC 2011 data families, - Smart Ration Card holder families, - J-Form holder farmers, - Small traders registered with Excise & Taxation Department, - Small & Marginal Farmers, - Accredited & Yellow Card Journalist and Construction workers registered with Punjab Building & Other Construction Workers Welfare Board.

The Scheme covers 1579 treatment packages that include hospitalization expenses, daycare surgeries, 3 days pre and 15 days post-hospitalization expenses and treatment of newborn child.
What is the maximum limit of provided health insurance ?
Health insurance cover of 5 lakh rupees per family per year.
Who can get the benefits?
All the eligible resident of Punjab.
How to apply for the scheme?
This is an entitlement-based scheme and no enrolment process for the families who are identified by the Government of India.

Can those families whose names are not on the list avail the benefits under AB PM-JAY MMSBY?
In this phase, no additional new families can be added under AB PM-JAY MMSBY. However, names of additional family members can be added only in case of families of J-Form holder farmers, families of registered Small traders and families of registered construction workers based on the family identification document produced by the Beneficiary at the point of contact. The family is defined as under: “Beneficiary Family Unit of Department Families will comprise of the Head of the Family/Main Member and any of his or her following relatives, irrespective of whether or not they are dependent or residing with the Main Member:
