Step 3: On the registration page, fill in all the mandatory fields (name, mobile number, gender, and captcha) and click "Verify Mobile Number." Step 4: A verification code will be sent to your mobile number. Enter the code and complete the registration. Application Step 1: Visit the Official Website of "Madhya Pradesh Lok Sewa Guarantee Portal". Step 2: Enter your User ID, Password, and Captcha Code, and click "Login." Step 3: Click on "Apply Online Service" and select the scheme you intend to apply for. Step 4: In the scheme&39;s online application form, fill in all the mandatory fields and upload all the mandatory documents in the specified format and size (fields marked with are mandatory), and click "Submit". Note 1: The incident must be reported within 48 hours to the nearest Forest Range Officer, either verbally or in writing. Note 2: The dead or injured animal should remain at the site for verification by the forest authority.
