Step 2: In the application form, fill in all the mandatory fields, paste a passport-sized photograph (signed across, if required), and attach copies of all the mandatory documents (self-attested, if required). Step 3: Submit the duly filled and signed application form along with a wage slip or appointment letter from the employer, verifying 90 days of construction work. If unavailable, a certificate from a local authority (such as a councilor, executive officer, or Panchayat Secretary) can be submitted to verify employment. Step 4: After submitting the documents and payment, the worker will receive an acknowledgment receipt confirming their registration.
