> For the Student Beneficiary - The beneficiary must be a student studying in all Primary, Secondary, and Higher Secondary schools of Gujarat. > For the Applicant/Claimant - The applicant must be the successor or the disabled beneficiary themselves in case of permanent disability. - The applicant must appear in person with the original identity card. - The applicant must submit the written application in the prescribed format to the local Nodal/Competent Officer. - The application must be submitted within ninety (90) days from the date of death or accident.

Application Process

Step 1: The applicant must obtain the necessary application forms and annexures through the Principal of the concerned school where the student was studying. Step 2: The applicant must fully and comprehensively fill out the prescribed application form and gather all mandatory supporting documents to substantiate the claim. The application must be accompanied by the original Identity Card. Step 3: The applicant (the heir in case of death, or the beneficiary in case of permanent disability) must submit the completed application along with all required proofs to the local Nodal/Competent Officer of the controlling office of the relevant department. This submission must be done in person (not via post) within ninety (90) days from the date of the accident or death. Step 4: The Nodal Officer is responsible for fully checking and verifying the application and must forward the application, along with a certification in the prescribed format, to the Office of the Director of Insurance or the Insurance Company. This forwarding must occur within thirty (30) days of the Nodal Officer receiving the application. Post-Application Processes Step 1: Resolution by Insurer The Insurance Company or the State Insurance Fund must decide on the claim approval or rejection within sixty (60) days of receiving the application with complete and satisfactory information/documents. Step 2: Claim Closure Timeline If the applicant fails to provide all requested documentation and fulfillments within the stipulated time, the claim will be closed automatically. The claim will also be permanently closed twelve months after the date of the accident/death, and no reconsideration request will be accepted thereafter. If a claim is rejected, the beneficiary or their heir must first submit a representation to the designated Review Committee within thirty (30) days. Legal action against the Review Committee&39;s decision can only be pursued after filing the review, and the legal/quasi-judicial/judicial jurisdiction is exclusively in Gandhinagar. Claim inquiries or representations must be made only by the applicant or beneficiary themselves, excluding any third parties like agents or lawyers.

No. Insurance compensation is payable only under one single category, even if the beneficiary is covered under multiple schemes simultaneously under the overall framework.

What is the mandatory time frame within which the necessary application form and all supporting documentation must be submitted to the local Nodal Officer?
